New Patient Registration

registration1The doctors welcome new patients who live within our practice catchment area. You will be asked to complete a registration form and health questionnaire and provide photographic proof of identity and proof of address, ie utility bill.  

Following registration it may take some time for your medical records to arrive from your previous GP.  To ensure continuity of care, you will be offered an appointment with our practice nurse who will take details of your past medical history, any medication you are taking and carry out a simple health check.

Medical treatment is available from the date of registration. 

You can get a registration form and New patient assessment form from  the receptionist. Please also bring photographic identity and a utility bill showing your current address when you attend the surgery to register
